A flexible case management/support
service is provided to people living in their own homes in Burlington,
Acton, Milton and Georgetown. Case managers work one-on-one with individuals and meet with them
in various settings such as the client's home, coffee shops, or
work place. Services - which are tailored to individual needs - include:
assisting people to access community services, to expand education,
to gain employment, to overcome addiction problems, to improve
life skills, to problem solve, to improve social skills, and to
budget to meet basic needs on a low income. Staff also provide
counselling, crisis intervention and advocacy. Intensity of service is based on need, for example, some clients
may need two to three visits a week, while others may only require
one visit a week. Others may only need one visit a month to enable
them to maintain their healthy lifestyle.
